2009-10 Flu Shot Information
We have seasonal flu and H1N1 vaccine still available. Please call to schedule an appointment.
Before you come to the office for an H1N1 vaccine you MUST do the following:
1. Register your child at: www.h1n1vaccine.odh.ohio.gov
2. Print out your registration form
If you fail to bring in your registration form you will not be able to get the vaccine.

Our phones are answered by our staff at the following times:
Monday through Friday 9am to 12noon, 1:30pm to 4:45pm
Saturday 8am to 9:30am
All other times the messages will be automatically forwarded to the Doctor "on Call". You can be assured that 24/7 coverage is available.
Specialties
We provide general medical care from birth to the age of twenty-one. We are available for ill visits as well as school/camp/sports physicals, behavioral concerns, initial allergy evaluations, and general preventative care.
